Explore the principles and mechanics of value-based care, including accountable care organizations (ACOs). Learn to select the right ACO for your practice and discover the crucial role of clinical interventions in ACO success. By the end of this webinar, you will be able to:
  • Understand the principles of value-based care
  • Describe the mechanics of accountable care organizations (ACOs) and how they fit into the health care ecosystem
  • Ask the right questions to identify the ideal ACO for your organization
  • Discuss the importance of clinical interventions such as annual wellness visits, transitional care management and advance care planning in ACO success
